Enrollment Information

Summit Middle School is located at 1703 NW 150th, Edmond, OK  73013.  This is between Western and Pennsylvania on 150th street.

To enroll a student in Edmond Public Schools, you must be the student's legal guardian or an adult who has assumed care and custody of the student. In either case, you must reside within the district boundaries.

You will need to bring the following information with you when enrolling:

  1. Mortgage or Deed to home OR Lease for home/apartment

     The student's name must be listed on the apartment lease.

 2.  Current Utility Bill (Gas/Water/Electric)

     For new utility service, the utility companies will provide a statement to parent/guardian showing service has been established at the new address, or you may provide the cut-off from your previous address and then a utility bill within 30 days.

 3.  Immunization Record (Required by the State of Oklahoma)

    5 DTP/Dtap  4 Polio  2 MMR  2 Hep A  3 Hep B  1 Varicella or history of disease

 4.  Legal State Birth Certificate (not a hospital copy)

 5.  In cases of divorce: Parent must show custody/guardianship.

      Only the parent with custody, living in the Edmond District, may enroll a student.

 6.  In cases of guardianship, legal papers must be signed by a judge.

 7.  Withdrawal Form*

       (From previous school - during school year for withdrawal grades.)

 * The name, address, phone and fax number of the previous school attended, unless the previous school was in Edmond. Special Education or Gifted/Talented records, if applicable, are helpful in ensuring the proper placement of the student.

   Custodial parents/guardians who cannot provide the necessary information to verify their address because they are residing with a friend or relative on a temporary or permanent basis are required to complete a Residency Verification form. The form requires both the Edmond resident and the custodial parent of the student be present when signed. Both the Edmond resident and the custodial parent must also provide valid identification.
